To all parents of students attending El Centro Elementary Schools in El Centro, CA. An H1N1 Pre-Registration form has been sent  to all schools today and it is very important that you return this form as indicated below as soon as you receive it from your child to ensure your he or she is considered for the clinic.

A todos los padres de familia que tengan hijos que assisten a las escuelas del Distrito Escolar de El Centro en El Centro, CA. Una forma de pre-registro ah sido enviada al las escuelas el dia de hoy y es muy importante que regresen esta forma como es indicado abajo de este mensaje en cuanto su hijo reciba esta forma. Esto es necessario para que su hijo e hija puedan ser considerados para la clinica. 

Only Pre-Registered students will be receiving the vaccination. These vaccination clinics are scheduled to offer the 2009 H1N1 nasal spray and shot.  A limited supply of vaccine is available for each school site.  T he selection process for the vaccination clinic will be determined by lottery. You will be notified if your child is selected.

►  Registration forms must be returned to school by 9:00 a.m. on Thursday
November 12^th, 2009.   Please return the bottom portion of this form to your child's
school in order for your child to be considered for the clinic.    

The vaccine will be provided at NO COST and will be provided to students who are:

·   Enrolled in schools below only.
De Anza
Desert Garden
Harding
Hedrick
Washington
Ballington
Kennedy Middle School
Wilson JR High
King
McKinley
Sunflower
Lincoln

·   Are not sick. If your child has a fever, please do not attend the clinic.

·   Accompanied by a parent or guardian.  

Please bring only pre-registered children to the clinic.

Solamente los estudiantes que están pre-registrados recibirán la vacuna. Estas clínicas de vacunación están programadas para ofrecer la H1N1 2009 inyectable o roció intranasal. Hay una cantidad limitada de la vacuna disponible para cada escuela. El proceso de selección para la clínica de vacunación será determinado por lotería. Se le avisara si su niño/niña es seleccionado.
